Authorize Seattle Town Car Services Seattle, WA. to charge my credit card in the amount stated below as the TOTAL once my reservation is confirmed. I understand that this charge is non-refundable and can not be canceled at any time. I understand that after signing this Authorization Form and entering credit card information in the appropriate fields, my credit card will be charge whenever Seattle Town Car Services Seattle, WA. decides to do so. I also understand that due to acts of God, mechanical problems ,traffic problems, or any other problems Seattle Town Car Services Seattle, WA. is not responsible for not being on time or having to send replacement limousine .
I agree not to ask for the charge back (due to problems occurred) at any time from my Credit Card Company or my Bank.

Seattle Town Car Services Seattle, WA. RULES &REGULATIONS

1.No alcohol consumed by anyone less than 21 years of age. Violation may result in early termination, no refunds.
2.NO SMOKING !!! We will charge $100 per incident.Any burn holes will be charged $ 500. No Exceptions !!!
3.Clean-up fee for vomit is $500.
4.All tolls and parking are not included in rental rate.
5.$100 clean up fee for excessive matter or any gum on the carpet floor.
6.All unruly persons will be dropped of a sole discretion of the driver & no refunds.
7.All overly drunk persons who may vomit during the drive, will not be allowed to enter the limousine.
8.We are not responsible for any lost, damaged or stolen property.
9.NO ILLEGAL DRUG OF ANY KIND.
10.Overtime will be billed in half hour increments.
11.In a case of cancellation NO MONEY WILL BE REFUNDED!!!
12.Customer agrees that overtime will be billed at the contracted rate and agrees  to pay any overtime charges.
13.Customer agrees that replacement Limousine may be substituted in the event of any major mechanical issues, limo involved in accident, etc.
14.All entertainment including cds, dvds,etc. is to be provided by the client.
15. Any broken parts inside or outside of the limo  will be charged minimum $ 500.

I AGREE WITH ALL ABOVE CHARGES,AND FURTHER AGREE TO PAY ANY AND ALL DAMAGES MY PARTY MAY CAUSE DURING THE RENTAL PERIOD.
I ALSO AGREE TO ABOVE RULES AND REGULATIONS.ALL DAMAGES ARE SOLE DISCRETION OF THE DRIVER.
